Fear 01

"I'm terrified I'll feel everything."

This is the most common reason adults avoid the dentist for years. The fear isn't irrational — it's a memory of a time when you weren't numb enough, and nobody noticed.

How we address this

Nitrous oxide and oral conscious sedation — two layers before we begin.

We use a protocol-driven approach: nitrous oxide (laughing gas) takes the edge off within three minutes of inhalation. For procedures requiring more depth, oral conscious sedation — a single tablet of triazolam taken 60 minutes prior — produces a twilight state where you remain responsive but have no memory of the procedure. You won't feel the needle. You won't feel the drill. The ceiling tiles will soften.

"I can't handle needles. And I gag on everything."

Two separate problems that make routine care feel impossible. Most dental offices don't have a protocol for either. We've built one.

How we address this

Topical anesthesia first. Always. Then a 30-gauge needle you won't feel.

We apply 20% benzocaine gel for 90 seconds before any injection — eliminating the sting that's actually the worst part. Our 30-gauge needles are the finest available in clinical dentistry. For gag reflex, nitrous oxide is clinically proven to suppress the hyperactive gag response. For severe cases, acupressure point P6 (wrist) and a single point of focus instruction reduces gag sensitivity by 40–60% in our patient population. X-rays are taken with a digital sensor, not film, reducing the size and duration of placement by 70%.

"I had a bad experience. I don't trust dental offices."

Dental trauma is real and physiological. Your nervous system learned a threat response. We don't argue with that — we work within it.

How we address this

IV sedation for full-mouth reconstruction. No memory. No compromise.

For patients with significant dental trauma or those requiring surgical extractions, implant placement, or full-mouth reconstruction, IV sedation administered by a board-certified dental anesthesiologist produces deep conscious sedation. You remain breathing independently, but you will have no memory of the procedure. We complete in one appointment what might otherwise require six. Dr. Elaine Marsh has performed over 2,400 IV sedation procedures with zero serious adverse events. Before any IV procedure, we conduct a 30-minute pre-operative consultation — the only appointment where we ask you to talk about the past.

From people who also cancelled
every appointment before this one.

I hadn't been to a dentist in nine years. I had three extractions, two implants, and six crowns done in a single day. I remember nothing after the IV went in. I woke up and it was over.

My daughter has SPD. Every dental visit was a crisis. With nitrous and the stop-signal protocol, she sat through a full cleaning for the first time. She was eight years old.

Pediatric sensory — nitrous protocol

I'm a surgeon. I understand anesthesia intellectually. I still couldn't sit in a dental chair without a panic response. Dr. Marsh didn't dismiss that. She just explained what she was going to do, and then she did exactly that.
